What this website asks for

The inquiry form asks only for your name, general coverage topic, and either a phone number or email address. When you submit it, that information is stored in the website’s hosted lead database so Jameel can respond and manage your request.

Traffic and campaign information

The form also records the page you used, referring website, language, and campaign tags in the URL. This helps us understand which search and outreach efforts lead to real inquiries. Your name and contact information are not sent to Google Analytics through this form.

What not to send

Do not include Social Security numbers, Medicare numbers, medical records, diagnoses, banking information, tax returns, government identification, policy documents, or other sensitive information in the website form or an ordinary email.

Permission to contact

Submitting an inquiry gives Jameel Habib permission to respond about the coverage topic you chose. Consent is not a condition of purchase, and you may ask not to be contacted again.
